How it Works

1.  Fill out the enrollment form on-line and hit submit button.

2. Your doctor will receive a short form to fax back to us verifying
your information.

3.  Within one week of receiving the completed form back from your
doctor, your monitor and testing supplies will be shipped to your
door free of charge.

4.  Enrolled patients initially receive a blood glucose monitor and a
three month supply of test strips, lancing device, lancets, and control
solution.  A three month supply will be shipped to you automatically
thereafter.  A new monitor will be supplied at no cost to the patient
as needed.

5.  Each year your doctor will be sent a new form to verify your
information.  You will not need to fill out any new forms.
